Clinical Study on 92 Cases with Congenital Hypertrophic Pyloric Stenosis


Abstract
The study is a clinical analysis of 92 cases with congenital hypertrophic pyloric stenosis, treated surgically at the Department of Surgery, Pusan National University Hospital, from July 1984 to June 1994.
@ES The results obtained were as follows:
@EN 1) Age distribution was 1 week to 18 weeks, the most prevalent age group was between 4 weeks and 5 weeks, and the ratio of male to female about 2.8:1. Among 92 cases, first-born babies were more than half (56 cases). Milk feeding was in 55
cases,
breast feeding was in 32 cases, nad mixed feeding was in 5 cases.
2) Vomiting occured in 2 weeks to 6 weeks in 64 cases. Non-bile stained, projectile vomiting was noted in all 92 cases, visible gastric peristasis was observed in 68 cased, and palpable mass on right upper quadrant of abdomen was felt in 82
cases.
The
average duration of th symptoms was 3 weeks. The shortest onset of symptoms was 4 days, and the longest was more than 9 week after birth.
3) On laboratory data at the time of admission, anemia was found in 15 cases, and jaundice in 12 cases. Hyponatremia (below 134 mEq/L) was observed in 62 cases, hypocalcemia (blow 3.9mEq/L) in 64 cases, and hypochloremia (below 96mEq/L) in 62
cases.
The combined anomalies were detected in 8 cases, and they were right inguinal hernia (2 cases), right undescended testis (2 cases), Down syndrome (2 cases), malrotation of intestine (2 cases). The characteristic findings of upper gastrointestinal
contrast study (80 cases) and ultrasonography (12 cases) were shown in all of the patients.
4) All 92 cases were surgically treated with Fredet-Ramstedt Pyloromyotomy, There were 3 cases of duodenal perforation, but were no death or incomplete pyloromyotomy.
